Form DS-2019

Form DS-2019 is the Certificate of Eligibility for Exchange Visitor Status Form that you will need to bring to the nearest United States Embassy or Consulate to begin the process of obtaining your J-1 visa. As your program sponsor, Van Andel Institute is responsible for entering the DS-2019 information into SEVIS, and providing you with the SEVIS-generated DS-2019. Please fill out the form provided on this site to send us the information we need to provide you with a SEVIS-generated DS-2019.

You should carefully review your DS-2019 when you receive it. The spelling of your name on your DS-2019 should match the spelling of your name on your passport. If your name is spelled incorrectly on your DS-2019 or any of the other information is listed incorrectly, please contact Margie Hoving immediately.
